Definition
The Nutritional Risk Screening tool is used to identify patients at risk for malnutrition. An initial screen assesses BMI, nutritional intake, weight loss and severe illness. If the initial screen indicates risk, the final screening scores patients from absent (0) to severe (3) in two areas, nutritional status and severity of disease. The two scores are them summed, with an additional point being added for patients over 70 years of age. Patients with a total score of 3 or greater are recommended for nutritional support.
