Each level is indicated using a traffic-light metaphor with written labels, color, position, and images. For analysis and reporting, each response level is recorded on a 0-3 ordinal scale, with excellent = 3, good = 2, fair = 1, and poor = 0. Higher scores indicated excellent service.
