Definition
Diastolic pulmonary artery pressure (dPAP) is used with pulmonary artery wedge pressure (PAWP) to help distinguish post-capillary pulmonary hypertension from combined post- and pre-capillary pulmonary hypertension associated with left heart failure. The diastolic pressure gradient (DPG) is calculated as dPAP minus PAWP, and is an alternate to the transpulmonary pressure gradient (TPG), which is calculated as the mean pulmonary artery pressure (mPAP) minus PAWP. An elevated DPG or TPG indicates that the increase in pulmonary pressure is not solely due to increased PAWP (post-capillary), but rather has a pre-capillary component as well, as reflected in an dPAP/mPAP increase out of proportion to PAWP.
