Definition

When evaluating newborns for sepsis, it has been found that the ratio of immature neutrophils to total neutrophils (I/T) in the peripheral blood can be a useful indicator. Several parameters are used to predict infection including a low white blood cell count (WBC), a low absolute neutrophil count (ANC), and a high immature neutrophil to total neutrophil count (I/T). Because of the expected variability of white blood cell counts in the first four hours after birth, it is recommended that the neutrophil count be done after the baby is four hours old. Low ANC indicate the highest risk for sepsis regardless of the I/T. High I/T levels only indicate a high likelihood of infection if the ANC is low. However, the effect of the I/T at various levels of ANC is less consistent. When the I/T is less than 0.15, the likelihood of infection is low regardless of the ANC.[PMID: 24503598]
