Definition
The heparin assay measures the patient's actual circulating concentration of unfractionated heparin in a sample of fresh whole blood by using titration with known quantities of protamine in individual assay channels. The channel whose quantity of protamine neutralizes the heparin in the test sample will have the shortest clotting time, and the concentration of heparin is calculated based on the known amount of protamine in that channel. The heparin concentration is used along with the activated clotting time in the cardiac OR to give a more complete picture of the patient's heparin anti-coagulation status. Using this test, the clinician/perfusionist knows how much heparin is present, and how much additional heparin is needed during the procedure, and the amount of protamine sulfate that is required to reverse the remaining heparin in the patient at the end of the procedure. This term was created for, but not limited in use to, the submitter's Medtronic HMS Heparin Assay test kit.
