Definition

This term is used to report the risk score for moderate to severe (grade 3A or higher) acute cellular transplant rejection in heart transplant recipients with stable allograft function. The test assesses the expression of 11 genes (ITGAM, FLT3, IL1R2, G6B, PF4, WDR40A, MIR, ARHU, PDCD1, ITGA4 AND SEMA7A) using PCR with RNA isolated from peripheral blood mononuclear cells, and the gene expression data is then used to calculate a score that ranges from 0-40, with a lower score indicating a lower risk for rejection. In the Cardiac Allograft Rejection Gene Expression Observational (CARGO) study, patients who were at least 12 months post-transplant and who had a score less than 30 were very unlikely to have grade 3A acute cellular rejection at the time of testing (negative predictive value 99.6%).
