Definition

A medication treatment plan (MTP) document is generated when a health care professional (e.g. medical specialist, general practitioner) adds a medication to the medication treatment plan of a patient. The document includes information about a patient's medication treatment plans [LOINC: 77604-7] as well as patient demographics, health care provider information, and authorization information. The MTP document can contain all medications the patient was supposed to take in the past as well as those that are current and active (i.e. should be taken by the patient). The information can be used to make an intake plan for the patient. The plan in itself has no dependency on prescriptions or dispenses, which are related to the logistic process necessary to get the drugs when specific procurement rules apply. This term was originally created for, though not limited in use to, the IHE Medication Treatment Plan profile in the Community Medication Prescription and Dispense set (CMPD).
