Definition
The Oncology Treatment Plan and Summary document provides a limited summary of a patient's care during and/or after treatment and continue to serve as a form of on-going communication to augment the coordination of care between health care providers while a patient is receiving care. Providers involved in the care of the patient may include the patient's primary care provider, medical oncologist, surgical oncologist, and radiation oncologist. This term was created for, but not limited in use to, the HL7 Implementation Guide for CDA, Release 2®: Clinical Oncology Treatment Plan and Summary. This guide was derived from the American Society of Clinical Oncology (ASCO) Breast Cancer Adjuvant Treatment Plan and Summary. However, the templates in this document are not restrictive and can be applied to other types of cancer.
