Definition

A request by a pharmacist (dispenser) for a formal prescription from a prescriber (e.g. physician). The prescription requests legitimatizes the dispensing of therapeutic goods following an instruction (e.g. through phone conversation or standing written agreement) from a prescriber. It has essentially the same contents as a prescription document except the pharmacist fills it out as a template for the doctor to sign.
